Service requests
In order to obtain an authorisation for the purchase of veterinary medicinal products, the owner of agricultural animals or the educational and scientific establishment shall:
- enter into a contract regarding the provision of veterinary medical services with a veterinary medical practice authority or a veterinary medical service provider (which does not work in a veterinary medical practice institution);
- ensure that the responsible practising veterinarian complies with the requirements specified in the regulatory enactments regarding the protection of animals used for scientific purposes, if this contract is entered into by an educational and scientific institution;
An application shall be submitted to the PVD, which shall specify:
- the name or surname, address, telephone number, e-mail address and address of the animal owner or educational and scientific establishment, if different from the registered office;
- the registration number of the holding and herd, if the application is submitted by the animal owner;
- the registration number of the PVD in the register of breeders, suppliers or users of experimental animals and the species of experimental animals, if the application is submitted by the educational and scientific establishment;
- the name, address of the veterinary medical practice institution and the given name, surname and veterinary medical practice certificate number of the practising veterinarian, who will continuously perform operations with veterinary medicinal products if the contract has been concluded with the veterinary medical practice authority;
- the given name, surname and veterinary medical practice certificate number of the provider of veterinary medical services, if the contract is concluded with the veterinary medical service provider, which does not work in the veterinary medical practice institution;
- the duration of the contract;
- the grounds for the purchase of veterinary medicinal products, attesting this by the signature of the responsible practising veterinarian and the stamp of veterinary medical practice.
An application may also be submitted by e-mail, signed with an e-signature.
Documents may also be submitted using the institution's e-address.
The applicant shall bear the costs related to the verification of the actual place of activity. The service charge is specified in the price list.Service Processing
The PVD shall, within 10 working days after receipt of the submission, evaluate it and, if necessary, inspect the actual place of operation of the permit applicant indicated in the application.Receipt of services
The PVD shall register the authorisations on the list of applicants for authorisation if the following conditions are fulfilled:
- the holding, herd and animals are registered and labelled in conformity with the requirements of the regulatory enactments regarding the registration of animals, herds and holdings and the identification of animals, if the requirement is applicable;
- the applicant for authorisation shall carry out holding of livestock for the production of products of animal origin or for other agricultural purposes and the number of animals in the herd shall not be less than 150 units of bovine animals if the applicant for the authorisation is the owner of the animals;
- the applicant for a permit shall engage in the rearing of animals intended for testing or use them in accordance with the regulatory enactments regarding the protection of animals used for scientific purposes, if it is an educational and scientific institution;
- the applicant for a permit has entered into the contract referred to in Sub-paragraph 14.1 of these Regulations;
- the competent practising veterinarian complies with the requirements specified in regulatory enactments;
- within the last 12 months, the PVD has not determined in the activities of the applicant for authorisation and the responsible practising veterinarian the violations of the regulatory enactments regulating the circulation of veterinary medicinal products and the maximum permissible levels of medicinal products, which could endanger human or animal health.
The PVD shall refuse registration on the list if the applicant for authorisation and the competent practising veterinarian have failed to comply with the above requirements.
The PVD shall take a decision to exclude the holder of the authorisation from the list of beneficiaries if:
- the examination shall identify a violation of the regulatory enactments regulating the circulation of veterinary medicinal products, which may endanger human or animal health;
- in foodstuffs of animal origin produced by the holder of the authorisation, residues of the veterinary medicinal product shall be established in quantities exceeding the maximum residue limits or substances prohibited for use in food-producing animals;
- in food-producing animals belonging to the holder of the authorisation, substances which are prohibited from being used in food-producing animals shall be
- the contract referred to above does not apply or the competent practising veterinarian does not comply with the requirements;
- the number of animals in the herd for more than six months does not meet the requirements;
- the holder of the authorisation is excluded from the register of breeders, suppliers or users of experimental animals;
- an application for exclusion from the list of authorised recipients has been received.
